This is relevant to those working in LTC facilities within Fraser Health Authority. The 2018 background is the most current.

V4_Jul3_2022 is FHA's attempt to combine the residential and acute care form into one.  Assessment can be done by the MD (for acute care  or LTC), or by the nursing staff at residential care facilities.  Signature stamp now included.
