I was asked to rewrite and update this form from 2011. There were some necessary upgrades. As well there is not much room under valvular heart disease for the comment lines. I have therefore made these hidden unless one of the three boxes (Aortic, Mitral, or Other) is checked. The comment box will then appear with a yellow background. Once it has been filled and you have moved away from this box, the background will become transparent again. If you save and re-open the form, the input fields are present if the box has been checked, but absent if the box has not been checked. The backgrounds will revert to transparent if the form is not new (has been previously submitted). JohnR.
