Created by Herb Chang. For the HPV self screening project in BC. Updted by Adrian Starzynski to include PDF and fax options.
A slightly different version of the At Home HPV screen was created for our site as we were piloting the new procedure for the past year. Since there is a Province-wide roll-out, I have added additional references for HPV screening, some of which is still labelled draft. This version (V4_Dec2_2023) has the Tickler Loop Closer feature.
