Clinical Frailty Scale

This tool was developed by Dr. Rockwood at Dalhousie. It has been further refined as a "Frailty Index".

To capture the results of this eForm you must add FRAI as a measurement.

Go to Administration>System Management>Customize Managements>Add Measurement Type

Type=FRAI ; Type Description=Frailty Index ; Display Name=Frailty Index ; Measuring Instruction=Score from 1 to 9; Validation= Numeric Value: 0 to 10
