Pacific Orthopedics and Sports Medicine Centralized Patient Referral (POSM)

This eFORM started as a Shared Care Project for the North Shore Division of Family Practice.  It was a rapid access orthopedic referral site.  There have been several versions since, and now the latest was inspired by work with the First Nations Primary Care Network.

The only glitch was the absence of a return contact number, which had to be added "after the fact".
