Sick Note: BC College of FP and BC Family Doctors version
This is the BCFP and BCFD's approved "generic sick note" eForm. If you have an office logo, it can be added to the top left corner of the eForm. You will have to modify the HTML in the appropriate sections and upload the png of your logo.
